Forms & Documents
Documents allow you to collect information, obtain signatures, share agreements, and gather approvals from clients. Businesses can create reusable document and form templates, then send them to clients for completion.
Doulado supports both structured forms for data collection and documents that require review and signature.

Where to Find It
Templates can come from multiple sources, making it easy to reuse existing documents instead of creating everything from scratch.
Templates can be managed in two places:
- Global Templates maintained by the Doulado team
- Business Templates created and managed by your business
Global Templates
Global Templates are pre-built forms and documents created by the Doulado team. These templates can be used as a starting point for your own paperwork.
To browse available templates:
- Go to Templates and click on the Paperwork side tab.

Click New Document or New Form.

Browse the available template gallery and choose a document template.

- Hover your cursor over a template and click Customize.
- This will create a copy in your business template library, allowing you to make changes without affecting the original template.

The next page will show you a zoomed out version of the template. Click Use this template to continue.

Fill out the necessary details and customize the template by adding fields.

- Click Save & Close.
Any customized template becomes your own business template and can be edited at any time.
Template Availability
Some global templates only appear when they are relevant to your business.
Service-Specific Templates

Templates may be linked to specific service categories, such as Maternal Health. These templates will only appear if your business uses matching service labels.

Affiliate Templates
Some templates are provided through affiliate partnerships and may be automatically available to businesses referred by a specific affiliate organization.
Data Sharing
If your business is connected to another business through Data Sharing, templates from the connected business may also be available when creating new documents or forms.
This allows teams and partner organizations to share paperwork resources and maintain consistency across businesses.
