Lead Forms
Overview
Lead forms are how a business (team or personal business) can allow clients to create profiles for themselves, using either a link directly to the lead form page, or by embedding the lead form into their website.

Where to Find It
The list of lead forms can be found by navigating to the Clients tab → Clients → Lead Forms (Button).

Page Overview
This page hosts lead forms that may be customized to collect different types of clients, leads, or services.

Tile Contents
Each tile is a separate lead form, which shows a preview as well as an actions menu. Clicking the tile will allow you to view a larger preview and copy the links.

Actions Menu

The actions menu allows you to:
- Edit the lead form title, description, and other details
- Add labels for insurance and payer filtering
- Duplicate the lead form
- Delete the lead form
