Lead Form Creation
Doulado calls a new client or contact form a Lead Form. You are able to have multiple different lead forms for varying needs or service types.
Each form has its own direct link and you can embed the code and use it to share with potential clients or put it directly into your website.
Lead Forms are a valuable tool for getting information from new clients or allowing them to sign up for your services.
Each time a lead form is submitted, a new client profile is created within Doulado in a Lead status.

- If you are a solo provider, you will get an email that you have a new lead.
- If you are on a Team account, the owners will receive an email to alert you of a new lead.
Follow these steps to create a new lead form:

Navigate to the Client tab, and click on the Lead Forms button at the top right of the main client list

Select New Lead Form to begin creating your form

A new window will open where you can enter the required information to start your form

- Name: Provide a name for the lead form (Doula Facing)
- Title: Add a title that best represents the purpose of the form (Client Facing)
- Redirect To (on Completion) (optional): Specify where users should be directed after submitting the form, such as a website or an external booking link.

Confirmation Email: Choose whether to send a confirmation email to the person filling out the form. Check the box to enable this feature, ensuring the user receives an email once the form is submitted, example lead form confirmation email below.

- Email Sent (on Complete): This email will be sent to the person who filled out the lead form.

Intro/Description: This section will be visible to the public. Use it to describe the purpose of the form to potential clients or provide additional instructions. It is optional, and will just show title if this space is left blank.
Example: Thank you for reaching out about our doula services. Please fill this form out completely, and we will be back to you within 48 hours. In the meantime, feel free to browse our website to learn more about us. Thanks so much!
- Once you’ve completed all the initial details, click Save to finalize your lead form.

After saving, you can choose to open your new form and customize the questions.
Each lead form automatically has basic questions to gather information from potential clients such as name, phone number, email, address, partner contact information, etc.
You can use the lead form as is, or customize the questions to suit your business!
If you open a lead form, you can view the direct link and embed code at the top. To customize the form, select customize at the bottom.
